We appreciate you choosing our office for your hearing needs.  We look forward to your upcoming appointment. 

As a courtesy, we file claims to your insurance company.  If you are unsure what your insurance benefits are, we are more than happy to check benefits on your co-insurance or deductibles with at least a week’s notice with provided insurance information.

If you have Medicare (red, white and blue card) as your primary insurance, it is required to have a referral from your primary care doctor.  Medicare requires that you contact your doctor’s office to have them fax the referral for the “hearing evaluation” to 423-641-0955.  If we do not receive the referral, Medicare will not pay for the hearing test.   If it has been a year since your last audiogram and you feel like your hearing has changed, an audiogram may need to be performed and a referral is required by Medicare.

We do require a confirmation at least 24-48 hours before the appointment to hold this time for you.  The confirmation can be via text, call or email.  We will send out several reminders until we receive a confirmation.  If we do not receive a confirmation, we will offer this time to someone else and reschedule your appointment. 

Our office hours are Monday through Thursday, 8am-4pm, Friday 8am-12pm. 

Regarding COVID-19:  we follow CDC guidelines.  At this time, a mask is not required and is optional.  If you are immunocompromized and wish for our staff to wear a mask in your presence, please call our office prior to your appointment advising us.
